Summer Registration Forms

Please fill out and submit via email to spgoutofschoolcare@gmail.com the following forms to complete your child’s summer program file. You may also pick up these forms in person or download and print:

• pick up : sign out : photo consent
• emergency card
• activities and excursion waiver

Please also submit the following:

1. Your child’s immunization record, as required by licensing (scan or photo works)

2. A wallet-sized photo to attach to their emergency card, as required by licensing
