Registration and Forms

SPG Currently has a long waitlist. Please Email spg to be placed on the waitlist. Your child will be placed in order of the date of the email.

Every September, the first ten spaces go to new Kinders, then current members expanding care needs, then new members Grades 1 – 7.

Once your child has been offered a space, please fill out the following forms to complete your child’s file.

You may submit forms via email to spgoutofschoolcare@gmail.com

Please also submit the following:

1. Your child’s immunization record, as required by licensing.
2. A wallet-sized photo to attach to their emergency card, as required by licensing.
3. Void cheque for auto-debit of program deposit and monthly fees.

You may also pick up these forms in person or download and print.

Note: Summer, winter and spring break participants are not required to complete a parent agreement form or long registration form.
